Fall Prevention

Falls are a complex problem with multiple causes and risk factors. Preventing falls in hospital is not easy, but there are many things we can do to help reduce the risk. Some hospitals may routinely use some or all of the following strategies to prevent falls.

Reducing hospital readmissions especially those that result from poor inpatient or outpatient care has long been a health policy goal because it represents an opportunity to lower health care costs, improve quality, and increase patient satisfaction at once.

Stroke

The transition home after a stroke can be a stressful one. Apabilities have diminished, sometimes quite substantially so that significant assistance is needed. Our goal is to assist our clients in regaining as much of those capabilities as possible.

Alzheimer’s & Dementia Care

Our caregivers on the dementia team receive ongoing training to work with Alzheimer’s and Dementia cases. People with Alzheimer’s Disease cannot control what is happening to them. We strive to learn new ways to relate to the person with Alzheimer’s.
