To families, residents and staff,
It is our goal here at Hearts & Hands Post-Acute Care and Rehab Center to be transparent and notify every one of the facilities current COVID status. Hearts & Hands is following all the latest CDC recommendations and guidance to make sure our residents and staff are safe. Hearts and Hands works closely with a variety of vendors and the local health department to ensure the facility has an adequate supply of PPE.
Currently we are postponing visitation based on Santa Cruz County’s guidance. The facility is currently in response-driven testing which means we are testing Health Care Personnel and Residents every 7 days until we reach 14 days with no new positive cases. When no new cases are identified in a 14-day period Hearts & Hands will move into screening testing which includes testing Health Care Personnel Weekly and residents who leave facility for regular appointments and or show symptoms. If you have any questions, feel free to call the facility and this page will be updated daily to reflect the facilities current situation.
